Referral Process

Due to the increasing number of Managed Care plans in our practice, it has become imperative that our patients take an active part in the referral process. When requesting a referral, please follow the steps listed below:

  1. Understand how your insurance policy works. Not all plans require referral from the Primary Care Physician (PCP).
  2. If this is a problem that Dr. Chandra Britt Armstrong or Dr. Kristy Levarity-Conley has not seen you for, please schedule an appointment. A referral may not be necessary.
  3. The initial referral to a specialist must be generated by your PCP. You will be provided with a list of doctors in your insurance plan's network, or you can look online for doctors in your network.
  4. It is the patient's responsibility to inform the office of any appointments when seeing a specialist, including any visits to urgent care facilities or to the emergency room. Please allow our staff up to 72 hours to process all referrals. Specialists will not see you without a referral, or you may be billed for services. If you wait until you get to your appointment to call for the referral, our staff will not be able to provide you a referral on that day.
  5. When calling to request a referral, please have the complete name of the specialist, their telephone number, your appointment date and your reason for referral. Please check with your insurance plan or specialist's office to ensure that the doctor is in your insurance plan's network.
  6. All referrals have a limited number of visits, so please contact our office. When referrals are requested the day of or without adequate notice, we cannot guarantee that the referral will be made in time for the appointment.
  7. When seeing a specialist, any procedures or surgery should be pre-certified by your specialist, not your primary care provider (PCP). However, we'll be happy to assist you in the process.
  8. Please be sure to take your insurance card with you to your appointment.
