About Us

At Dekalb Family Medicine on Candler (DFMOC), we make it our mission to provide the highest quality care to all members of the family in a pleasant atmosphere, delivered by a helpful, caring and friendly staff. Our goal is to be one of the best healthcare providers in the county.

We would like to take this opportunity to thank you for choosing Dekalb Family Medicine on Candler to provide your primary care needs.  Our mission is to create an environment that promotes and supports good health by assisting the community in discovering the full wealth of positive long term life style habits.  The Staff and Providers dedicate ourselves to providing quality health care in a compassionate partnership with our patients.  We will educate, deliver and coordinate the highest quality of comprehensive patient-centered health care by helping patients of all ages navigate the health care system in a respectful and accountable manner.

We need you to provide us with a complete medical history and information about your care obtained  outside of our office.  We will ask you to periodically update information and share any changes at each visit.  Our physicians are Chandra Armstrong, MD and Darren Harper, MD.  We also have nurse practitioner Tonita Washington, DNP who has a doctorate degree in nursing.  We ask that you select one of the providers within the office with whom you are comfortable and can expect to see at the majority of your visits.  However, occasionally your primary provider may be unavailable and you may need to see another provider in the office to address your medical concerns.  We use an electronic medical record system and ask that you provide us with an accurate email address to use only for the purpose of communicating lab results through a secure internet site called Relay Health, or for us to provide you with important information from our practice.

Our office hours are 8:00a.m. – 5:00p.m. Monday – Friday and our phone lines are open from 8:30a.m - 12:00p.m. and 1:30p.m. – 4:30p.m. We are closed for lunch from 12:00-1:00p.m.  If you need to call for clinical advice during office hours, we ask that you call our main phone number (404) 289-4556 and listen for the prompt that addresses your need.  We ask that all prescription requests are left on the prescription line and prescriptions will be addressed within 24 hours of the request if left Monday – Friday before 4:00p.m.  Any prescription requests after 4:00p.m. will be handled the next business day.  If you need a referral, we ask that you allow 72 hours to process the referral.  Please leave the name of the provider, the date and time of the appointment on the referral line.  If you need urgent clinical advice during office hours and the phone lines are not open, someone from our answering service will take the message and try to contact someone in the office to assist you in an Urgent situation only.  If you are experiencing an emergency please call 911. 

Lastly, if you need urgent clinical advice after hours, the answering service will page the provider on call and the will contact you and give you further instructions.  Also, we do provide care at our Stonecrest office location on the 1st, 2nd, and 4th Saturdays of the month from 9am – 12pm for patients.
